
IS YOUR HEALTH CARE SYSTEM ACCESSIBLE? 
Federal and state laws prohibit disability discrimination and require access and accommodation. People with disabilities must be able to use all health care facilities, services, and programs.
The Disability Rights Legal Center in Los Angeles (DRLC), recently received funding from the California Endowment to support their work involving the Respect-ABILITY Coalition, a statewide group of disability rights and service organizations.
DRLC worked with the California Foundation for Independent Living Centers and other coalition partners to develop a survey to assess the accessibility of health care service providers in California.
Please help us obtain this important health care information by completing the survey. The survey may be taken and submitted on-line at www.respectabilitycoalition.org or a PDF or WORD version may be downloaded and returned by mail or fax. This survey is also available in Braille, large print, and computer disk, as well as in Spanish.

Your participation in this survey will help us to develop a fact sheet about the accessibility of health care services across California. We will report our findings at the Respect-ABILITY Conference in March 2008 and use those findings to work on developing policy solutions for greater access to quality health care for people with disabilities throughout California.
